Expert judgement needed :-)
This image is done with a bag of tricks from the HtciP book. Is it fully realistic? If not, what needs to be improved? Many thanks in advance!

I think the perspective of the lens is wrong, it still looks like ita floating in front of the face, instead of being part of it.

I like the concept Robert - very Terminatorish!

The issue here is that the new eye assembly does not blend with the face for a number of reasons.

The metal would be recessed into the skin and would follow the contours of the face. Shadowing is key to realism.

A little liquify on the top portion of the metal to curve those lines to produce a socket shape would help. The lower lines should follow the contour of the cheek. Also I think the eye unit is just too large when compared with the real eye. The direction of the lens seems wrong to me for some reason. I think we would be looking from more of a side angle and the lens should be recessed into the socket more.

The face photo is pin sharp whereas the metal effect around the electronic eye is soft in appearance.

There is strong light hitting the metal from the left, but this is not reflected in the fingers and the overall lighting on the face, which is relatively even. The lens I referred to earlier would be casting a shadow on the surround and a shadow would help it blend in.

Possibly not what you wanted to hear, but the concept is good and with some more work it could be great.

I made this image as a "prototype" and will rework it from scratch to include all feedback!

No one has mentioned that the lens is pointing straight at us but the head (and presumably the other eye) are looking to the model's right?

I can't wait to see this when it's finished!

Thank again, the lens direction is easy to fix. I have made the lens in Illustrator, and I can tilt it in 3D mode... I have to wait until the weekend before I can hide away for a couple of hours!

As a very quick go I think adding perspective to the eye lens gives it more realism. What I did was make a eliptical selection of the orginal lens copied it to a new layer (Ctrl+J) and Free Transformed it into perspective - I selected the lens and Ctrl+Alt nudged using the right arrow key until it developed the 3D look. I duplicated this layer and scaled it down to give a Telescopic Lens look.

The original eye lens was cut away from the face to a new layer and transformed to fit the new perspective. Some Dodge and Burn on the girl's face and some shadows added to the new telescopic lens and some more tweaking get us to the picture here.

Thirty seconds' work:

New layer.
Draw the droplet with the Polygonal Lassoo Tool. Thus a selection.
Fill it with red.
Layer properties: Bevell & Emboss, and a little Drop Shadow.
Don't forget to finish with the little white highlight (brush tool) to make it glisten.

Robert, if I amy add, you may want to desaturate the subject a little. The lighting in the background doesn't appear to have much yellow/orange in it. The subjects edges look a little too crisp too. You could lightly use the blur tool around the edges.
