The main elements of the drawing and new building were 3D modelled in a matched perspective. Once you have that carefully set up the perspective takes care of itself. Same goes for the reflections. Flip the 3D upside down and move it 'below ground'. This creates the 'structure' for the reflections which are then treated in Photoshop.
I'd love to have a suitably clever techno babble answer to that but I just found him and his ladder, exactly as they are, by pure lucky chance!
